Supplemental Report
Supplemental Report is used to supply entries / information in the Certificate of Live Birth, Certificate of Marriage, Certificate of Death, and Certificate of Fetal Death, which were inadvertently omitted when the civil registry document was registered.
Who May Avail
Any interested party covered by Civil Registration Law (Act. No. 3753), its rules and regulations, Memorandum Circulars issued by the PSA and Data Privacy Act of 2012 (R.A. 10173)

Processing of Documents
Present Official Receipt, receive Transaction Slip and returns after the 7 days processing of registration
